Will someone please help me with this letter as soon as possible?
Since this deceased is my husband’s niece, should I just put my husband’s niece? My husband will need me to accompany him since he has cancer, diabetes and other ailments. The only days that I have missed from work except for medical purposes or for funeral leave. My supervisor does not hassle me about my work, but I want to make sure that I sound courteous. Also, who should I CC copies of this letter to: I work a small regional correctional facility? My supervisors are as followers:

Do you think this should also be CC'd to the Sheriff (I feel that it is relatively minor)

I am requesting December 22nd through the 28th off due to the unexpected death of my niece in Chicago, Il. As the only next of kin, my husband and I need to make the funeral arrangements there.
This is short and to the point. They don't need all the extraneous details unless they follow up and ask for them.

I need to accompany my husband as he has serious illnesses including cancer and diabetes. Due to the distance in traveling, I request that you please grant me leave on Wednesday, Thursday and Friday (December 25-28th) to take care of funeral arrangements."
Why do you need to tell a sheriff about this, your supervisor can do so it necessary.

It looks like you are trying to get an extended Christmas vacation.
This assumes that you'll spend Christmas day on the road to Chicago. You should supply your niece's name so your employer can verify your claim. A quick check of the death notices would support your reason to request time off.
You'll need to sell it better than that, because funeral arrangements can be made from anywhere using telephone or internet. Will you later be asking for more time to attend memorial services and burial? Such things don't usually happen withing a single day.
You don't happen to be Jewish... are you? That would really help!
