    I'm not going to help because it's YOUR work to do, but I will point out that what's written between #s 3 & 4 is wrong.  "Will you mind ________________ (lend) me your pen?".  It's "WOULD you mind...", not "Will".  Only Spanish speakers say "will" where it's "would", so I doubt that an English textbook would print that. If they did, it's not a good source to learn from and you should point out that error to your teacher.
