What is the cause of dysfunction within a system?

there usually are only two causes ... first is poor system design.  second is poor selection and training of personnel to operate the system.  the first is frequent when a committee is in charge -- which includes every legislative body throughout history.

Either the "feed back" is not being taken and fed into the control part of the system at the right places or it is being ignored. That is assuming that the system has a feed back loop to maintain control.

Possibly the wrong perimeters are being used for a control standard.
