Could North Korea actually get a missile or nuclear warhead to the States?

First off, in order for NK to deliver a nuclear payload to the United states they would need a nuclear warhead small enough to be fitted to an intercontinental ballistic missile, which they dont.... yes they have the nuclear capability and the missile capability, however they dont quite have the technology needed to merge these two and create a functional ICBM. Now they are within years of reaching this milestone, however there are also other factors including the fact that the more threats they make, the more they are cut off from trade.

For decades the U.S.A. was easily able to square off against the Soviet Union, so do you think a country like North Korea, with a 100% active propaganda machine churning out garbage is to believed, a country where mass starvation is commonplace and a teenage dick tator who has never had sex with a real woman yet is to be feared?

Assuming a North Korean warhead incredibly hit the U.S. it would in all probability be someplace on the West coast, la or San Fransisco consistent with probability. To answer your question: No ! and that's not because the USA are "monitoring the sky" as people have quoted here. Their nuclear devices are just not miniaturised enough to be embedded in one of their missile warheads..
