Yes, but they are getting very rare. grab a photo if you can. here soon it will be the only proof we ever had them at all. As for where and whos, i recently came across a picture and caption stating the semaphore was on the old Pere Marquette Ry. ( CSX ) in Michigan. This was a dated picture of 2 years ago.

Not to my knowledge. Not any Class 1's anyways.I work on a line that runs from Texas into New Mexico that still had a number of semaphore signals but they were removed when the Union Pacific took over the Southern Pacific.I believe the year was 1998 that the last ones were gone.I was told that they were bought by tourist railways and museums so it's possible some are still being used somewhere.

I believe theres still 1 or 2 left on the CSX Main line which is the route of the former Monon Railroad. I believe the route runs from Louisville, Kentucky to Chicago. But thats the only one left on a main line.
There might still be a small number left on disused sidings; I did see one myself outside of chicago 7 years ago.
If you are planning to visit it and take pictures, I suggest you do it as soon as possible, before they take those down too (If there still there).
Good Question = 1 Star.
Edit:
I did some research and found out that I was right; and its more specifically in Indiana.

The last ones I saw were at Cass Scenic in West Virginia. Since it's not connected to any class 1 roads I doubt if it would count. I'm not sure if Strasburg still uses them or not. I do know they still use ball signals at Promintory Summit in Utah for demonstrations.
Here's some reference links:
